You MUST be 18 years old with valid photo identification for tattoo services. Anyone under 18 MUST HAVE an ID and a notarized parental consent form, as required by Florida State Law. Consent forms can be downloaded from http://www.floridahealth.gov/ENVIRONMENTAL-HEALTH/tattooing/.  Please print the "Minor Consent Form" and have it filled out and signed by a parent or legal guardian in front of a notary and bring it with you to your appointment.
